The Dental Choice team of dentists, hygienists, insurance coordinators, and other staff members is proud to provide compassionate and comprehensive dental care. We strive to make visits to our offices as comfortable and relaxing as possible for the entire family. We offer general, cosmetic, restorative, and children's / pediatric dentistry at our locations in Edmonton and Calgary, Alberta. Our family and children's dentistry practice bills insurance companies directly so that patients don't have to worry about paperwork. In order to help expedite your claim quickly and to help ensure you are given the maximum coverage that you're eligible to receive, please provide us with your insurance information by filling out this form.

Dental Questionnaire

Please fill out our dental questionnaire to let us know what type of dental issue you are having. You may attach it and email it to us at info@dentalchoice.ca. Someone from our offices will respond to you as quickly as possible.
